PURPOSE To maintain at-risk children in their home and community.

SERVICES Case management, family preservation, in home services for families, mental health

OFFERED therapy, wrap-around services, development of an individualized service plan in

collaboration with other agencies.

REFERRAL SOURCES Written referral from DJJ, DSS, GCMH, Bd. Of Ed. and approval of

Local Coordinating Council.

HOURS OF SERVICE On call as needed 24 hours, 7 days a week. Office hours: Mon. thru Fri.,

8 AM to 4:30 PM.

FEES FOR SERVICE None

WAITING PERIOD Minimal: for referral to be WAITING LIST No

approved.

HANDICAPPED ACCESSIBLE? Yes

ACCOMMODATIONS FOR DISABLED? Yes, as needed; hearing impaired call one week in

advance.
