| PURPOSE | To provide screening, assessments and interventions to address health and eductional |
| needs of children ages birth to 3 years who are experiencing developmental delays or at |
| risk for developmental delays. |
| SERVICES | May include: speech therapy; physical therapy; case management; and diagnostic, |
| OFFERED | evaluation and health services as identified on the treatment plan. Training, counseling, |
| education, support and special instruction for parents. |
| REFERRAL SOURCES | Self-referral by phone, physician's referral letter, social services worker, |
| health care worker, other agency or person. |
| HOURS OF SERVICE | Services are scheduled as needed. Office hours: Mon. thru Fri., 8:00 AM |
| to 4:30 PM. |
