REFERRAL SOURCES Referrals must come from: Bd. of Education, Dept. of Juvenile Justice,

Dept. of Social Services, or GC Mental Health.

HOURS OF SERVICE On call 24 hours, 7 days a week. Office hours: Mon. thru Fri., 8 AM to

4:30 PM.

FEES FOR SERVICE None for families served.

HANDICAPPED ACCESSIBLE? Yes, handicapped ramp to enter office.

ACCOMMODATIONS FOR DISABLED? Yes, as needed; hearing impaired call one week in

advance.

LOCATION OF Services are primarily delivered in the consumer's home.

SERVICES

TRANSPORTATION Consumer transports self, agency provides transportation as needed, worker

visits home, or GTS will transport for a small fee.

ELIGILIBITY CRITERIA

AGES SERVED Children/Youth, birth to age 18.

GEOGRAPHIC AREA SERVED Must be a resident of Garrett County, MD.

INCOME GUIDELINES N/A

DOCUMENTS REQUIRED Written referral from one of the four agencies listed above.

OTHER Child must be determined to be at risk for placement out of the home.

VOLUNTEER Volunteer opportunities are available for special projects. BROCHURE No
